Codex CLI agent is ignored "Request approval" instruction

Issue Description

While working in the Codex CLI agent we began coding the “new feature” change immediately after drafting the OpenSpec proposal, skipping the required approval step from openspec/AGENTS.md (“Request approval: Do not start implementation until proposal is approved”). This undermines the spec workflow and puts unreviewed changes at risk of landing in the codebase.

I encountered the same situation as you.

When I push a special comment to the Codex CLI agent to mark this option as mandatory, it works as expected, or almost so.

I encountered this issue myself, but was able to work around it by entering "Request approval: Do not begin implementation until the proposal is approved" as the first argument in the proposal.
