Add title parameter to task_update tool - #22

Allow renaming tasks via task_update by providing a title parameter.
Updates the H1 heading in the markdown file and syncs to the SQLite
index via upsert_task. Also exposes the title field through the REST
API's PATCH /api/tasks/{id} endpoint.
